When it comes to male infertility, varicocele is one of the most important — and most correctable — conditions a man can have. Yet it is also one of the most overlooked. Many men live with a varicocele for years without any symptoms, unaware that it may be silently undermining their fertility. Others are diagnosed only after a couple begins struggling to conceive.

What Is a Varicocele?
A varicocele is an abnormal enlargement of the veins within the scrotum — the network of veins that drain blood away from the testes. These veins, known as the pampiniform plexus, function like a natural cooling system, helping to regulate testicular temperature. When these veins become enlarged and dysfunctional, blood pools in the scrotum instead of draining efficiently, raising the local temperature and triggering a cascade of harmful effects on sperm production.
The condition is essentially varicose veins of the scrotum — and just as varicose veins in the legs cause poor circulation and discomfort, varicoceles impair the delicate environment the testes need to produce healthy sperm.
Varicoceles are almost always found on the left side (around 90% of cases), due to the angle at which the left testicular vein drains into the renal vein. Bilateral varicoceles (affecting both sides) occur in approximately 10% of cases.
How Common Is Varicocele?
Varicocele is far more common than most people realise:
- Found in approximately 15% of all adult men
- Present in 35–40% of men being evaluated for primary infertility (first-time difficulty conceiving)
- Found in 75–81% of men with secondary infertility (difficulty conceiving after a previous pregnancy)
These statistics make varicocele the single most common surgically correctable cause of male infertility.
How Does Varicocele Affect Fertility?
The testes require a temperature approximately 2–3°C below core body temperature to produce healthy sperm. This is precisely why the testes are located outside the body — nature’s own temperature regulation system. A varicocele disrupts this system in several important ways:
Elevated Testicular Temperature
Pooling blood in the dilated veins raises scrotal temperature. Even a modest increase of 1–2°C is enough to impair sperm DNA replication, reduce sperm production, and damage motility.
Oxidative Stress
Impaired blood flow increases the production of reactive oxygen species (ROS) — harmful free radicals that attack sperm cell membranes, impair mitochondrial function, and fragment sperm DNA. This is why varicocele affects all three major sperm parameters: count, motility, and morphology simultaneously.
Hormonal Disruption
Raised testicular temperature interferes with the Leydig cells responsible for producing testosterone. Men with varicocele often show lower testosterone levels and elevated FSH — a hormonal pattern reflecting reduced testicular function.
Sperm DNA Fragmentation
One of the most clinically significant effects of varicocele is elevated sperm DNA fragmentation — damage to the genetic material inside sperm. High DNA fragmentation increases the risk of fertilisation failure, poor embryo quality, failed IVF cycles, and recurrent miscarriage.
Grading of Varicocele
Varicoceles are graded based on size and detectability:

Symptoms of Varicocele
One of the most challenging aspects of varicocele is that many men experience no symptoms at all, and the condition is discovered only during a fertility workup. When symptoms do occur, they may include:
- A dull, aching pain or heaviness in the scrotum, typically on the left side
- Discomfort that worsens after standing or physical activity for extended periods and improves when lying down
- A visible or palpable lump above the testicle — sometimes described as feeling like a “bag of worms”
- A visibly smaller testicle on the affected side (testicular atrophy) — a sign of impaired blood supply and reduced function
- In some cases, reduced libido or fatigue linked to lower testosterone levels
It is important to note that pain severity does not correlate with fertility impact. A painless, subclinical varicocele can cause just as much damage to sperm quality as a large, symptomatic one.
Diagnosing Varicocele
Physical Examination
A clinical examination by a trained urologist or fertility specialist is the first step. The doctor palpates the scrotum while the patient stands and performs the Valsalva manoeuvre (bearing down as if straining) — which increases pressure in the scrotal veins and makes a varicocele easier to detect.
Scrotal Doppler Ultrasound
This is the gold standard for varicocele diagnosis. A high-resolution ultrasound with Doppler flow imaging:
- Measures the diameter of the pampiniform plexus veins (>3 mm is diagnostic)
- Detects retrograde blood flow — the hallmark of a varicocele
- Identifies subclinical varicoceles not detectable on physical examination
- Assesses testicular volume and identifies associated abnormalities
Semen Analysis
A comprehensive semen analysis will typically reveal the characteristic “stress pattern” of varicocele — a combination of reduced count, poor motility, and increased tapered or immature sperm forms.
Hormone Profile
FSH, LH, testosterone, and prolactin levels help assess the degree of testicular dysfunction caused by the varicocele.
Sperm DNA Fragmentation Test
Given varicocele’s strong association with DNA damage, this test is strongly recommended, particularly for couples with unexplained IVF failure or recurrent miscarriage.
Treatment Options for Varicocele
1. Microsurgical Varicocelectomy
This is the gold standard treatment and the most widely performed surgical approach. Under an operating microscope, the surgeon identifies and ties off the dilated veins while carefully preserving the testicular artery, lymphatics, and vas deferens. The subinguinal microsurgical approach offers the highest success rates and lowest complication rates.
Outcomes after microsurgical varicocelectomy:
- Improvement in sperm count, motility, and morphology in 60–70% of men
- Natural pregnancy rates of 30–40% within one year of surgery
- Significant reduction in sperm DNA fragmentation
- Improved testosterone levels in many men
2. Laparoscopic Varicocelectomy
A minimally invasive surgical option performed under general anaesthesia. Small incisions are made in the abdomen to access and ligate the testicular veins. Results are comparable to microsurgical repair, though with a slightly higher rate of hydrocele (fluid accumulation) as a side effect.
3. Percutaneous Embolisation
A non-surgical, radiological procedure in which a catheter is passed through the femoral vein to the testicular vein, and a coil or sclerosing agent is used to block the abnormal vein. Performed under local anaesthesia with no surgical incision. It has a shorter recovery time but slightly lower success rates compared to microsurgical repair.
4. Varicocele Repair Before IVF/ICSI
For couples already planning IVF or ICSI, varicocele repair prior to treatment can significantly improve sperm quality — leading to better embryo development, higher implantation rates, and reduced miscarriage risk. When Is Treatment Recommended?
Not every varicocele requires treatment. Our specialists consider treatment when:

Subclinical varicoceles detected only on ultrasound, with normal semen parameters, may be monitored rather than treated immediately.
What to Expect After Treatment
Sperm production is a continuous process taking approximately 74 days. This means improvements in semen quality after varicocele repair typically become measurable at the 3-month mark and may continue improving for up to 12–18 months post-surgery.
During recovery:
- Avoid heavy lifting and strenuous activity for 2–3 weeks
- Sexual activity can usually resume after 1 week
- A follow-up semen analysis is recommended at 3 and 6 months post-procedure
